Plaque psoriasis is a chronic autoimmune inflammatory disease driven by dysregulation of the IL-23/Th17 immune axis, which accelerates skin cell turnover so cells build up faster than they shed — forming thick, scaly plaques most often on the elbows, knees, scalp and lower back. It affects more than 8 million Americans and an estimated 125 million people worldwide, and accounts for roughly 80–90% of all psoriasis cases.[2] The conventional treatment path — topical steroids, coal tar, phototherapy, systemics and biologics — is genuinely effective and carries genuine trade-offs, which this guide sets out plainly. Gentle, steroid-free daily skin care has a supporting role in comfort and barrier care alongside that treatment. It is not a replacement for it.

What Plaque Psoriasis Actually Is — The Immune Mechanism
Psoriasis is not a skin problem with an immune component. It is an immune disease that shows itself on the skin. That distinction shapes how it is treated — and why treating only the surface gives temporary results.
In healthy skin, cell turnover runs roughly a month — new cells form in the basal layer, migrate to the surface and shed in an orderly process. In plaque psoriasis, an overactive immune cascade driven by the IL-23/Th17 pathway accelerates that cycle dramatically. New cells reach the surface faster than they shed, forming raised, scaly plaques. The same inflammation causes the redness, heat and itch — and extends systemically, which is why psoriasis is associated with psoriatic arthritis, cardiovascular disease and metabolic syndrome.[1]
IL-23 activates Th17 cells, which release IL-17 — a cytokine that drives keratinocyte hyperproliferation and chronic skin inflammation. The most effective pharmaceutical interventions available today — biologics such as secukinumab, ixekizumab and risankizumab — work by blocking IL-17 or IL-23 specifically.[1] That precision is exactly why they work well. It is also worth being clear about what botanical skin care does not do: no cosmetic formula blocks IL-17 or IL-23, and none of the products described later in this article act on this pathway. They work on the skin surface — comfort, scale, barrier — which is a different and more modest job.
Who Gets Psoriasis — Prevalence and Risk Factors
Psoriasis often first appears between the ages of 15 and 35, though it can develop at any age. A genetic component is established, with HLA-C*06:02 showing the strongest association, particularly for early-onset disease.[1] Genetics alone don't trigger it — environmental exposures, immune dysregulation and stress responses all play a part.
An earlier version of this article said psoriasis affects all skin tones "equally in prevalence." That is not what the data shows. The National Psoriasis Foundation reports prevalence of 3.6% in Whites, 2.5% in Asians, 1.9% in Hispanics and 1.5% in Blacks — while noting these figures may themselves reflect health disparities, including underdiagnosis and lower rates of dermatologic care among minoritized groups.[2] The honest version is more useful than the tidy one: reported prevalence differs, and part of that difference is very likely a measurement problem rather than a biological one.
Types of Psoriasis — Identification Guide
Plaque psoriasis accounts for most cases, but knowing the full spectrum helps in recognizing when something is psoriasis rather than another skin condition. None of the descriptions below are a substitute for a diagnosis — only a clinician can make one.
Plaque Psoriasis (Psoriasis Vulgaris)
Raised, inflamed, scaly patches — typically on the elbows, knees, lower back and scalp. Plaques are red or pink with silvery scales on lighter skin; darker, thicker purple or brown patches on skin of color. Can be localized or widespread.
Scalp Psoriasis
Present in a large share of psoriasis patients — flaking, scaling and inflammation on or extending beyond the hairline. Often co-presents with body plaque psoriasis. Needs different formats than body care — see our dedicated scalp psoriasis guide.
Inverse (Flexural) Psoriasis
Smooth, shiny, non-scaly plaques in skin folds — armpits, groin, under the breasts, genital area. Lacks the typical scale because friction and moisture prevent buildup. Often mistaken for a fungal infection. Skin here absorbs topicals more readily, which matters for steroid use.
Guttate Psoriasis
Small, drop-shaped lesions appearing suddenly across the trunk and limbs — often following a streptococcal throat infection, particularly in children and young adults. May resolve on its own or develop into chronic plaque psoriasis.
Pustular Psoriasis
Pus-filled blisters surrounded by red skin — not an infection, but requires medical evaluation. Localized forms appear on palms and soles. Generalized pustular psoriasis is rare and can be life-threatening. Any sudden widespread pustular eruption needs immediate medical attention.
Erythrodermic Psoriasis
A severe, potentially life-threatening form covering most of the body with widespread redness, scaling and shedding. A medical emergency requiring immediate care. Often triggered by abrupt medication discontinuation or infection — one more reason never to stop treatment without medical guidance.
Triggers — What Causes Psoriasis Flares
Psoriasis is not continuously active — it flares when triggers activate the immune cascade against a background of genetic susceptibility. Identifying personal triggers is one of the more useful things a person can do, and it works alongside medical treatment rather than instead of it.
Where Conventional Treatment Falls Short
The 2026 psoriasis landscape is more advanced than it has ever been. Multiple biologics are approved, IL-17 and IL-23 inhibitors achieve high clearance rates in trials, and steroid-free topical options now exist. For people who can access these treatments and tolerate them, this is real progress, and this article is not here to talk anyone out of them. But there is a gap between trial results and long-term real-world management, and it is worth naming.
Topical corticosteroids — effective, and hard to use forever
Topical steroids remain first-line for mild-to-moderate psoriasis globally, and they work. The difficulties come with the long-term use a chronic condition invites. Tachyphylaxis — a diminished response with repeated use — is documented,[4] and long-term use is associated with skin atrophy, telangiectasia, striae, and in rare cases enough systemic absorption to cause Cushing-like effects.[6] None of this means steroids are bad medicine — it means they need medical supervision over the long run.
Coal tar — old, useful, and unpopular with patients
Coal tar is one of the oldest psoriasis treatments in use. It reduces scaling and slows cell turnover. It also stains, smells strongly, causes irritation and photosensitivity, and contains polycyclic aromatic hydrocarbons — a long-standing safety discussion in the literature. The clinical consensus is that therapeutic coal tar at regulated concentrations is acceptable for its intended use; the practical problem is that people struggle to keep using it, which is a compliance issue rather than a safety verdict.
Biologics — highly effective, not universally accessible
For moderate-to-severe psoriasis, biologics targeting IL-17 and IL-23 are transformative. The barriers are cost, injection or infusion schedules, monitoring requirements, and warnings around infection risk. Published estimates of annual cost commonly run into the tens of thousands of dollars before insurance, and actual out-of-pocket cost varies enormously by coverage — so treat any single figure with caution.[3] Patient hesitancy about biologics is itself documented in the clinical literature.[7]
An earlier version carried a quotation about the industry "embracing clinical botanical solutions" that had no source attached to it, and a claim about a National Psoriasis Foundation recommendation that we could not verify at NPF. Both are gone. If we can't point to who said something, it shouldn't be in quotation marks on a page about a serious disease.
Topical Steroid Withdrawal — What Is and Isn't Established 2026
Topical Steroid Withdrawal (TSW) describes a group of adverse effects reported after stopping prolonged use of medium- to high-potency topical corticosteroids: burning pain, marked redness, swelling and widespread skin peeling.[8]
Here is the honest state of it, because this topic attracts a great deal of overstatement in both directions. It is real enough that a national medicines regulator acted on it: the UK's MHRA introduced new labelling on topical steroids in 2024 and reminded clinicians that reactions have been reported following long-term use — generally six months or more — of moderate or stronger potency steroids.[9] At the same time, the MHRA noted there has been little new information in the scientific literature and no accepted clinical definition. A January 2026 case series and narrative review describes TSW as an underrecognized and at times controversial diagnosis without standardized diagnostic criteria.[10]
One scope point that matters and is usually left out: most published TSW cases involve eczema, not psoriasis, and the MHRA's own advice singles out eczema treatment. That doesn't mean psoriasis patients are unaffected — the chronic nature of psoriasis does create pressure toward long-term steroid use — but the evidence base sits mostly elsewhere.
If you have been using topical steroids long-term, do not stop them abruptly. Rebound reactions after sudden cessation are exactly what the regulatory warnings describe, and abrupt discontinuation is also a documented trigger for erythrodermic psoriasis. Any change to a prescribed steroid should be planned with the doctor who prescribed it. A cosmetic skin care routine is not a taper plan and cannot substitute for one.
The Gut-Skin Axis — What the Research Does and Doesn't Show
The connection between gut health and psoriasis is one of the more active research areas in dermatology, and it is also one of the most oversold in marketing. Both things are true, so here is the line between them.
What the research reports: people with psoriasis tend to show reduced gut microbial diversity and lower levels of short-chain-fatty-acid-producing bacteria such as Faecalibacterium and Akkermansia compared with controls.[11] Separate work has examined intestinal barrier markers and their relationship to psoriasis severity.[12] This is a genuine and interesting line of investigation.
What it does not show: that any particular supplement changes the course of psoriasis. Association is not causation — reduced microbial diversity may be a consequence of systemic inflammation rather than a driver of it, and that question is not settled. No study has tested a Kapyderm supplement in psoriasis.
It would be easy to use the gut-skin research as a reason to sell supplements harder, and plenty of brands do. We would rather be accurate: the Kapyderm supplements in this range are food supplements, not treatments for psoriasis, and the gut research is a reason to find the topic interesting, not evidence that a capsule will clear plaques. If you want to act on this area, the conversation is with your physician — particularly about diet, weight and alcohol, which have the most support of anything discussed here.
Psoriasis on Skin of Color
How psoriasis presents, gets diagnosed and gets treated differs meaningfully for people with darker skin, and the dermatological literature has historically underserved this population.
How it looks different: on darker skin, plaques often appear purple, grayish or dark brown rather than the classic red or pink of textbook descriptions, and scaling can look ashy. The contrast between affected and unaffected skin differs, which makes standard severity scoring less accurate. These differences contribute to misdiagnosis and delayed treatment — and NPF notes explicitly that psoriasis is likely underdiagnosed among Black patients and others with skin of color because of these presentation differences.[2]
Post-inflammatory hyperpigmentation (PIH): after plaques clear, dark marks can remain for months. PIH is more pronounced and more persistent on melanin-rich skin, which means the visible impact of psoriasis continues after the active inflammation has settled.
Topical steroids can cause localized hypopigmentation, which is more noticeable on melanin-rich skin, and this is a legitimate thing to raise with a dermatologist when discussing treatment options. It is not a reason to avoid effective treatment — it is a reason to have the conversation. The single most useful step is being seen by a clinician who regularly treats your skin tone; asking directly how often they do is a fair question.
